The Six-Week Progress Check

A one-page decision form for the first check-in on a Tier 2 intervention.

Six weeks in, most Tier 2 groups have enough data to make a first decision. This form keeps that decision short and grounded: what the data shows, whether the intervention was delivered as planned, and what happens next.

One rule first: if the intervention wasn't delivered as planned, the decision isn't “not responding.” Fix the delivery, then check again. For a closer look, use the free Tier 2 Fidelity Check.

1What's inside

Two pages: a short guide, and a form you use once per student.

Your school's rules. Two blanks you fill in once: your fidelity threshold and your decision rule. Every school sets its own, and using the same rules on every form keeps decisions consistent from student to student.
The data so far. Six weeks of scores or ratings, with sessions delivered against sessions planned.
Before you decide. Four yes-or-no questions: delivered as designed, enough sessions, the same measure each time, and at least six data points.
The decision. What the trend shows, the decision, what changes, who tells the family, and the next check date.

2The four decisions

Continue. The student is on track toward the goal. Keep going and set the next check date.
Adjust. Some growth, but not enough. Change one thing: time, frequency, group size, or strategy.
Intensify. Little or no growth, and the intervention was delivered as planned. Add time, make the group smaller, try a different intervention, or bring the student to the SST.
Step down. The goal is met and holding. Move back to Tier 1 support and set a follow-up check so the gains don't slip.
Fill it in before the meeting. The person who delivers the intervention brings the scores and session counts. The team makes the decision together.

3Where it fits

The Tier 2 Fidelity Check answers the first question on the form in more depth. The 30-Minute MTSS Meeting gives the team an agenda for making the decision, and the Tier 2 Intervention Sampler shows the monitoring cadence and decision rule for nine interventions.

Most useful about six weeks after Tier 2 groups begin, and again at every check date after that.

Completed forms contain student information and should be handled according to your school's student-records and document-retention policies. Decision rules and fidelity thresholds vary by school and intervention โ€” set your own on page one.
